Contact Information Sheet 

On occasion, you may need to contact other club members, and vice versa. This form is to collect your contact info, and facilitate that need.    

We would like your complete information on file (e.g. emergency contact info, should something happen during a swim/bike/run workout), so please fill out the complete form.  We recognize that personal contact information is sensitive, and so only club members (and only those with a password to the Inner Circle) will have access to the information you provide.  It remains your decision as to what information you are willing to divulge to other club folks that access the directory page (checked box indicates you're ok about sharing that field, unchecked boxes indicate a wish for privacy).
